Meet the doctor

Dr. Jeremiah Cook was born and raised in Highland, Utah. He graduated summa cum laude with a Bachelor of Science degree in Biology from Utah Valley University where he met and married his wife Annilee. He received his Doctor of Dental Surgery degree from Baylor College of Dentistry where he graduated with honors and received the Quintessence award for clinical achievement in Periodontics. He completed his specialty training in Periodontics and implant dentistry at the University of Texas Health Science Center at San Antonio where he also earned his Master of Science in Periodontics. Dr. Cook is a Diplomate (board-certified) of the American Board of Periodontology. Dr. Cook has advanced training in cosmetic periodontal therapy, bone grafting, dental implants and IV conscious sedation anesthesia

Dr. Cook is a board certified periodontist who is an expert in his field and well known for his caring attitude and warm sense of humor. He has changed many lives by improving their oral health and smiles.

Dr. Cook and his wife have 3 children and reside in Prosper, TX. They have made Texas their home after spending a decade here. Dr. Cook is excited about being a periodontist and serving the community.
